A health record (sometimes referred to as medical record) should contain all the clinical information about the care you received. This is important so that every healthcare professional involved at different stages of your care has access to your medical history, such as allergies, operations or tests. Based on this information, healthcare professionals can make judgements about your care going forward.

To help improve the sharing of important information about your health there are two types of electronic health records for patients in Oxfordshire:

  • The Summary Care Record (SCR) - an electronic record available to authorised NHS health care staff treating you in urgent or emergency situations anywhere in England 
  • The Oxfordshire Care Summary - a single electronic view of specific, up-to-date, clinical information from your GP record and other records which may be kept to support your care in NHS organisations in Oxfordshire
