Report of independent investigation into the care and treatment of Nico Reed

A report of an independent investigation into the care and treatment of Nico Reed has been published today 27 June 2018 by Oxfordshire Clinical Commissioning Group (CCG).

Oxfordshire CCG commissioned the independent investigation following the death of Nico Reed, aged 23, at Barrantynes Supported Living Home in Chalgrove, Oxfordshire, in August 2012.

Nico had been living at the home since 2010 where he was receiving care and support for cerebral palsy and a learning disability. The home was run by the former Oxfordshire Learning Disability NHS Trust (also known as The Ridgeway Partnership).  Social care services were funded by Oxfordshire County Council.

In the autumn of 2015 Oxfordshire CCG commissioned the Health and Social Care Advisory Service (HASCAS) to carry out an independent investigation into the care and treatment Nico received during his life. This followed an inquest and an internal investigation into his death.

The report published today has identified five key areas of learning. These are underpinned by 19 recommendations which will serve as guidelines to provide better care for people with complex needs in Oxfordshire. 

Sula Wiltshire, Director of Quality and Lead Nurse at Oxfordshire CCG, said: “We wish to extend our deepest condolences to the parents, family and friends of Nico Reed. His memory is cherished by everyone who knew him. We thank the parents of Nico Reed who very generously gave up their time to take part in the independent investigation we commissioned. 

“The report was a thorough and detailed examination of the care and treatment Nico received throughout his life and particular his final two years, and the events surrounding his death. The detailed recommendations will be adopted in order to ensure people with complex needs and learning disabilities receive more effective and appropriate care in the future”
