Independent report on Ms A

An independent report on the care and treatment provided to Ms A by Oxford Health NHS Foundation Trust has now been published by NHS England.

Sula Wiltshire, Director of Quality and Board Nurse for Oxfordshire Clinical Commissioning Group said: “We welcome the independent report on the care of former patient of Oxford Health NHS Foundation Trust. Whilst the investigation concluded that the death of Mr X could not have been predicted or prevented we acknowledge the areas identified in the report that require action.  We will continue to work closely with Oxford Health to ensure they implement of the report’s recommendations and support improvements in the quality and safety of mental health services in Oxfordshire.”
