Locality – Oxford City – September 2015

Improving Access to GP Services

The Locality’s Patient Forum was given an update in July on the Prime Minister’s Challenge Fund pilot projects being developed for the City and Oxfordshire. Various schemes are being rolled out via GP Federations to help improve patient access to doctors’ services in the county. Forum members heard how Practice based Care Navigators will be launched in Oxford later this year. The scheme will help coordinate care for patients who are elderly, frail and just out of hospital, linking up practice teams with other health and care agencies.
